هذا التقرير ينشر لبحث اسباب الحوادث ومعالجتها لتلافي وقعها مره اخرى وقد اعد هذا التقرير لشهرين فقط لعام 2008 م Reported Incidents for the last 2 Months 26/07/2008 Reserve Deployment – Dunnotar Pilot Experience : 70 hrs 380 flights Glider Rating : DHV 2 General Conditions : Clear thermic day with lots of turbulence and dust devils Wind : Variable < 15km/h Description : Asymmetric collapse with incorrect and delayed input resulted in Spiral. Pilot deployed reserve at 50m and landed without injury. Pilot stated reasons as over confidence and inexperience. 22/08/2008 – Bambi Pilot Experience : Experienced Glider Rating : DHV 1-2 General Conditions : Clear thermic day with lots of turbulence Wind : Strong N turning NW Description : Pilot flew 1 flight in basically un-flyable conditions and landed after suffering numerous collapses. Wind turned to almost over the back. Walked back up and launched in gully to left of normal launch. Was gale hanging when he was not having collapses. Tried to top land and missed. Suffered a full frontal, over corrected and stalled the glider into the mountain. Casevac’d with suspected broken back, but was lucky to suffer minor bruises (internal and external). 23/08/2008 – Bambi Pilot Experience : Student Glider Rating : Old DHV 1 General Conditions : Clear thermic day with lots of turbulence Wind : Strong N / NE estimated at close to 30 with gusts Description : While other instructors suspended training, one decided it was a good idea to have female students ground handle. He was questioned about the sanity of this by numerous other pilots. His assistant lost his grip on one of the students and she became airborne and blown back. She was not in control of the glider and got dragged upon touching down. Casevac’d with suspected broken arm and complaining of back pain, but was lucky to suffer minor cuts and bruises. Will most probably never want to fly again!!! 04/09/2008 – Lions Head Pilot Experience : Student – Not yet licensed Glider Rating : DHV 1-2 General Conditions : Clear thermic day Wind : Moderate S - SW (Gusty) Description : Pilot came in for a tight landing at the Sea Point Promenade. Landing with the wind. Got dragged thru trees and dropped 3-5m breaking leg and tearing ligaments. Required surgery. 06/09/2008 – Sir Lowry’s Pass Pilot Experience : Very Experienced Glider Rating : DHV 2 (3) General Conditions : Clear thermic day Wind : NW 15 km/h Description : Pilot requested an anchor, and after pulling the glider up instructed the anchor to let go, but having turned the wrong way. Pilot tried to untwist, but was stuck in compression zone. Glider drifted back and rolled sideways accelerating pilot into ground. Suffered broken pelvis and arm/elbow. 06/09/2008? – Somewhere in Cape Town Area Description : Visiting Pilot had an Incident – No details reported. 13/09/2008 – Dumbe – PaulPietersburg Pilot Experience : Recently Licensed (04/2008) Glider Rating : DHV 2 !!!!! General Conditions : Clear thermic day with lots of turbulence Wind : E – NE 15-25 km/h, predicted gusty Description : Pilot suffered a collapse shortly after launch, most probably due to rotor, was not able to recover glider and plunged 30 – 40 m into the rocks below. Suffered severe fractures of right leg (top to bottom), right pelvis and right arm. Suspected internal injuries. Pilot in critical condition. Pilot was told by other pilots not to fly. 14/09/2008 – Laudium Pilot Experience : Student
Glider Rating : DHV 1-2 General Conditions : Clear thermic day with lots of turbulence Wind : N 15-25 km/h Description : Pilot suffered a 50% collapse, close to the hill, did not counter steer and impacted with the hill at high velocity. Broken pelvis, broken ribs, punctured lung. Pilot in ICU |

ياساتر مشكور عالتقرير وحسب ماورد فيه بأن اغلب الحوادث كانت بسبب سوء الاحوال الجوية وعدم ملائمتها للطيران ولكن الطيارين تجاهلو ذلك وحصل ماحصل . السبب الثاني هو الثقة الزائدة للطيارين . تحياتي |
